Guidelines
In order to provide limited
benevolent care for those families in financial need when faced with medical or travel
expenses for eye or tissue surgery the District 2-E1 Lions Tissue & Eye Bank Board of Directors
has established the following grant requirements.
- All grants shall originate through the local Lions Club.
- The local Lions Club will be
responsible for screening all requests and determining the need for financial assistance.
- The Lions Club making the request must be current in their
annual $1.00 per member contribution to the District 2-E1 Lions Tissue and Eye Bank when
making the request.
- Matching funds by the local Lions
Club will be required for all grants.
- Requests for amounts up to $750.00 each may be approved
with the Tissue and Eye Bank contributing $500.00, providing that matching funds of
$250.00 are available from the local club.
- The local Lions Club will submit the
request in writing, describing briefly the need, name of the recipient and enclose the
club's check for matching funds made payable to the District 2-E1 Lions Tissue and Eye
Bank. A check will be returned from the Tissue and Eye Bank to the local Lions Club made
payable to the recipient or relative, in an amount equal to the club contribution and the
matching grant
- A recipient of financial assistance
is eligible for only one grant.
